Change surname
General
Do you wish to change your surname or that of your children? Check first in the Ministry of Justice brochure ‘Naamwijziging van Justis’ if it is possible in your case. Justis is a department of the Ministry of Security and Justice.
If your situation described in this brochure, you can submit a request for name change to Justis.
Submitting an application
- Download the application form and fill it in.
- Check the Name Change brochure to find the code which applies to your situation. Use this code when filling in your application form.
- Attach the requested documents to the form. Send everything to Justis.
- You will be notified whether or not your name has been changed. You will receive an official decision by Royal Decree.
How long will it take?
Within 20 weeks, Justis will notify you whether or not your name has been changed. This may take longer if your application is incomplete.
Modification of your documents
- Has your surname been officially changed? It will also have to be changed in your proof of identity. You must apply for a new passport, identity card and driving licence.
- Make sure that you also notify all official authorities of your change of name. You do not need to
inform the municipal authorities separately. They will automatically receive a copy of the decision by Royal Decree.
Costs
Information about the cost of an application to change a surname can be found on justis.nl.
Justis is a website of the Ministry of Security and Justice. The ministry will let you know how much you will need to pay after it has received your application. You must pay this amount in one single payment.
What if the Ministry decides to reject your application? Or if you decide to withdraw your application? Your money will not be refunded.
Submitting an objection
If you disagree with the decision, you can file an objection with Justis within 6 weeks.
