Proof of life (attestation de vita)
General
Proof of life or an Attestation de vita are used to prove to the authorities that you are still alive. You can apply for these documents at the desk in the Town Hall.
We only work on an appointment basis. You cannot drop in without an appointment. You can make an appointment online or by telephone on (0224) 210 400. Appointments are free of charge.
Proof of life
A Proof of life is an excerpt from the Key Register of Persons (BRP) and is intended for use in the Netherlands. It contains the following information:
- Your name
- Your address
- A declaration by the Mayor that you are still alive at the time of issue.
You may authorise someone else to apply for a Proof of life for you. This authorised person must show proof of identity and bring a written authorisation and copy of proof of identity belonging to the applicant. It is also possible to apply for a Proof of life online.
Attestation de vita
An Attestation de vita is a proof of life document for use abroad. A translation may be attached. An Attestation de vita must be applied for in person. It is not possible to apply for an Attestation de vita online.
How long will it take?
If you apply in person at the Town Hall, the excerpt will be made while you wait. If you have applied online it will be sent to your home address. This may take several days.
Costs
It will cost €9.30.
A (demonstrable) application on behalf a Pension Fund is free of charge. You must be able to prove that you need a Proof of life/Attestation de vita for a Pension Fund. Usually, you will have received a letter from the Pension Fund on this subject. If you have a made an appointment at the Town Hall, you should bring this letter with you. If you apply for a Proof of life online, you can upload the letter and there are no costs involved.
